Everyone’s goal is to stay at home for as long as possible. I always add “safely” to that goal. Once we have that goal in mind, the work becomes easier.


I am here to work with you to create a safe, reasonable and financially possible plan for yourself, or if you are a caregiver, for the older adult in your life.


We will work together to open a dialogue about aging. We will discuss short term interventions and assistance, and look down the road for longer term planning as well.

How can I help?

I work with caregivers to understand disease processes and what we can do to prepare for what is coming next, especially when facing cancer or dementia diagnoses.

 

My knowledge of hospital systems and discharge planning means that I can guide you through the chaotic time of having a loved one in the hospital, and trying to sort out what kind of care they need following a hospital stay.

 

I have extensive experience with palliative, hospice and end-of-life issues and care, so when I say I am here for you for the entire journey, I truly mean through it all.

 

Every family, caregiver and older adult is unique and valued by me, and it is important that everyone feels respected and cared for at all times in this work together. I recognize and honor diverse families and caregivers, including chosen and found families.
